The Institute, having the mandate to regulate professional safety practice in Nigeria, among other responsibilities, as empowered by ISPON Act 2014 has continued to pilot the affairs of safety in the public and private sectors through world class safety events, training, certifications and membership registration, just to list a few.
This year 2021, our conference with the theme "Improving HSE Performance Through the Development, Application and Implementation of Effective Safety Case Across Industrial Sectors" is focussed on expanding the scope and application of HSE Case beyond the oil and gas sector as a strategic move towards improving risk management processes in all sectors of our industries.
The 40th ISPON conference will provide great insights to help organisations develop effective risk control methods, demonstrate adequacy of the controls and implement to protect people, assets, environment and reputation of the organisation.

The Institute of Safety Professionals of Nigeria (ISPON) was established in August 1980 and accredited vide an official Gazette of the Federal Republic of Nigeria No. 62, Vol. 101, by an Act of Parliament on May 19, 2014, as the professional body for the advancement and regulation of safety practice in Nigeria with the following objectives:
To organize and provide professional training on safety management;
To set standards of practice and determine the knowledge and skills to be acquired by persons seeking to register and practice as safety professionals and to improve on such standards from time to time in collaboration with relevant government agencies;
To do all such things as may be necessary to promote the advancement of safety profession in both the public and private sectors of the economy.
In furtherance of the noble objectives with which it is charged, ISPON utilises all available avenues with the aim of sharing common vision and drive for safety excellence in Nigeria and beyond through policy-advocacy, best practices and capacity-building.
The annual professional development conference (PDC) brings together intellectuals, seasoned safety practitioners, policymakers, consultants, researchers and scholars from all industrial sectors, organisations and parts of the world, and draws from the rich cross-sectional knowledge, wide experience and versatility of well-tested speakers, for the benefit of all.
This year’s annual Professional Development Conference (PDC) of ISPON, the 40th in series is billed for the month of October 2021 in Abuja-FCT. Actual dates and venue to be communicated subsequently. The theme of the conference is: “Achieving HSE Excellence through the Establishment of Effective Safety Case”.

A safety case is a document produced by the operator of a facility which Identifies the hazards and risks inherent in the facility operation, describes how the risks are controlled, describes the safety management system in place to ensure the controls are effectively and consistently applied to mitigate adverse occurrences from the hazards
Safety case is popular in the oil & gas industry BUT as can be seen from the definition above, is applicable across industry horizontals. So, our focus should not be only oil & gas-centric!

Leadership is considered a critical component in assuring the safe running of organizations. The prevailing workplace safety culture are mostly created and propagated through the existing organizational leadership. This cultural factor always influence behavior in every setting, whether at home, in the workplace or society at large. Similarly, this has a ripple effect on new joiners in the way they conduct themselves especially when out of sight or leadership is not visible. This presentation is intended to explore the gains of adopting visible and felt safety leadership as a bedrock of organizational excellence. Although the overall safety culture for most organizations have improved over the past 15 years, performance Indicators witnessed a flattened curve in recent years. This suggests that engineering controls, safety management systems, compliance to standards and rules have reached their limits and a new approach has become exigent. Therefore, adopting visible and felt safety leadership is paramount to understanding reasons behind unsafe behaviors exhibited by people regardless of their competences, experiences and qualifications. Company Z in focus is one of the major Oil and Gas company in Nigeria. This presentation is premised on statistics and baseline data pre and post adoption of safety leadership in the organization. Data sets includes but not limited to Incident reports & unsafe acts/conditions; causal leaning investigations to identify behavioral “system” patterns; safety culture/behavioral safety diagnostic surveys involving both leadership and frontline employees; focused group discussions & “commitment workshops”; performance monitoring through feedback/soundbites. The outcome revealed 17 “System Patterns” - a combination of behaviors, resources and structures that influence the humans operating in the system and has led to several significant and high potential incidents in the organization. Further investigation also revealed that reshaping or changing the narratives around these “system patterns” will lead to 80% reduction in significant incidents. More so, diagnostic survey analysis exposed differences in the perception of prevailing safety culture as well as opportunities for improvement such as respect, fairness, trust, openness, speaking up, empowerment, and interventions. The focused group discussions identified leadership development opportunities such as coaching, observing & engaging differently, safety refresh agenda involving “system pattern” reshape, Human Performance & learner mindset programs. A major incident is generally the result of several factors interacting in the system. It is true that a strong Safety Culture is not in itself an absolute guarantee for incidents not to happen, but “heartfelt & genuine care for people” provides a solid platform that reduces chances for complacency, omissions, violations and other unsafe behaviors. “People don’t care how much you know until they know how much you care” – this underscores the place of visible and felt safety leadership as the bedrock for organizational safety performance and excellence. ABSTRACT Safety is a considered top priority in every organization due to its significance in safeguarding human lives and property, especially in the high risk industries like the oil and gas, construction, mining, steel industry and manufacturing industries. It is important to establish and execute an effective safety management system to continually improve on the safety risks which can measured and monitored . The most famous result is the incident/accident pyramid, also known as the “safety pyramid” illustrates that for every reported major injury (resulting in a fatality, disability, lost time or medical treatment), there were 9.8 reported minor injuries (requiring only first aid). (W.H Heinrich, 1941; Frank E.Bird,1971). A few companies was analyzed to confirm that BBS, Near miss and Unsafe condition reporting reduces accident rates and improves HSE performance. Measuring HSE performance is a key tool to entrenching positive safety culture in an organization. Therefore it is very important to positively build and improve the culture of an organization to report incidents, measure and monitor their safety performance, be it small, medium or large scale industry because unsafe behaviours will surely lead to accident. Heinrich Domino model of accident causation states that 88% of all accidents are caused by unsafe acts of people, 10% by unsafe actions and 2% by “acts of God”. ( W.H Heinrich, 1931). The safety culture of an organization is the product of individual and group values, attitudes, perceptions, competencies, and patterns of behaviour that determine the commitment to, and the style and proficiency of, an organisation’s health and safety management. Organizations with a positive safety culture are characterized by communications founded on mutual trust, by shared perceptions of the importance of safety and by confidence in the efficacy of preventive measures. The construction industry in Nigeria is a dynamic and innovative one that delivers buildings and infrastructures for all aspects of commercial, industrial, domestic, energy, telecommunications, manufacturing, oil and gas sectors of the economy among others. It contributes to 3% of the Nigerian GDP, one-third of the total fixed capital investment and employs 5.7% (December 2019) of the total workforce in Nigeria. Most construction activities are transient as they are categorized as projects and involve activities like design, procurement, premobilization, actual construction (fabrication / building), finishing, pre-commissioning / commissioning, operation /hand over for use. Construction industry in Nigeria is faced with several safety challenges leading to various incidents and its assorted harms including injuries / fatalities to people, damage to environment, assets and reputation which occurs on a project-by-project and day-by-day basis. Accident is not caused by a single factor but a combination of factors which were not safely managed. The main causal factors of construction accidents are human driven and characterized by poor work interface designs, poor decisions, inadequate supervision, insufficient site management, poor equipment maintenance and management, use of incompetent workers, psychosocial factors including stress and fatigue, poor emergency preparedness and response, inappropriate personal protective equipment management, failure to use personal protective equipment’s and unsafe equipment used in construction works.DESIRE OBANAFE

This paper is based on a study of expert publications on emergency preparedness and response and how it affects HSE performance positively, through the application of the right tools, techniques and methodologies in emergency preparedness and response. This presentation is important because nobody expects an emergency or disaster -- especially one that affects them, their employees, and their business personally. Yet the simple truth is that emergencies and disasters can strike anyone, anytime, and anywhere. An organization and its employees could be forced to evacuate the company when least expected. Moreover, when it comes to emergency preparedness and response, there are usually a number of challenges such as command and control, the challenge of getting external assistance, coordinating effective evacuation, lack of equipment, lack emergency situational awareness by employees and customers and much more. It identifies parties involved in Emergency Preparedness and Response Planning, such as the corporate organization, Local Emergency Planning Coordinating Group, the local government, neighbors, NGOs and Local Emergency Response teams. The literature review helped to identify 10 excellent solutions in integrated Emergency Preparedness and Response Planning as follows: 1. Identify emergency response participants and establish their roles, resources and concerns. 2. Evaluate the risks and hazards that may result in emergency situations in the work environment and community and define options for risk reduction. 3. Have participants review their own emergency plan, including communications, for adequacy relative to a coordinated response. 4. Identify the required response tasks not covered by existing plans. 5. Match tasks to resources available from the identified participants. 6. Make changes necessary to improve existing emergency plans, integrate them into an overall workplace and community plan and gain agreement. 7. Commit the integrated plan to writing and obtain endorsement for it and relevant approvals. 8. Communicate final version of integrated plan to participating groups and ensure that all emergency responders are trained. 9. Establish procedures for periodic testing, review and updating of the plan. 10. Communicate the integrated plan to all stakeholders. ABSTRACT
The conventional Hazards and Operability (HAZOP) technique is well developed and applied to process systems design / operations, to identify potential deviations from the plant design intent and develop both engineered protective systems (main control system, safety instrumented systems, relief systems, etc.) and administrative controls (safety critical procedures) to manage the risks associated with the deviation. However, conventional HAZOP is blind-sided to the risks posed by human failures and reliability issues which are at the root of most accidents in the process industry. PHAZOP (Procedure Hazards and Operability) study methodology has been developed to fill this gap, complimentary to the conventional HAZOP studies. This paper aims to outline the PHAZOP technique, its timing, critical success factors, the aim is to create the awareness of the technique among Safety Engineers, Process Plant Design Engineers and Operations & Maintenance staff. Having attended both primary and secondary schools in the 70’s and 80’s respectively, he proceeded to Enugu state University of Technology where he obtained a Bachelor degree in Applied Biochemistry in 1997. He obtained an MSc in Analytical Chemistry from the University of Ibadan in 2005, a post graduate diploma in occupational safety and health in the workplace from the international training center of the International Labour Organization (ILO) in collaboration with University of Turin in 2008. Daniel later obtained an MSc in safety and risk management from Heriot Watt University, Edinburgh, Scotland UK. I also have a diploma in Quality management, lead auditor certifications for ISO 9001, 14001 & 45001 management systems. Read profileEngr. Martina Uzoamaka Atuchi is currently the General Manager, Joint Venture (JV) Oil Operations Division, of the National Petroleum Investment Management Services (NAPIMS), a Corporate Business Unit of the Nigerian National Petroleum Corporation (NNPC).
Engr. Atuchi is a graduate of Petroleum Engineering from the renowned Federal University of Technology Owerri (FUTO).
Over thirty (30) years, through impeccable dedication to duty and dint of sheer hard work, Engr. Atuchi rose through the ranks from a Graduate Engineer in the Kaduna Refinery and Petrochemical Company (KRPC) in 1990 to become the General Manager, Joint Venture Oil Operations Division, National Petroleum Investment Management Services (NAPIMS), where she currently oversees the operations of several Joint Venture (JV) companies, ensuring that the Federation extracts maximum value from its upstream oil and gas assets.
Her work experience has traversed the entire value chain of the oil and gas industry, serving in and leading various committees and projects to deliver value to NNPC and Nigeria.
